COMMUNITY OUTREACH

Responsibilities include the following:

  • Develops relationships within the community in order to educate and promote Among
    Friends Adult Day Health Care program to healthcare professionals and community
    members.
  • Identifies prospective individuals in need of Adult Day Health Care services through
    visits and presentations at community centers, county healthcare agencies, etc.
  • Develop and promote referrals based on community relationships.
  • Assist with in-home assessments, case management.
  • Complete intake process and guided tours at the facility.
  • Act as an advocate for participant concerns.
  • Develops, plans, coordinates and implements communication strategies, marketing, and
    public relation campaigns
  • Develop and manage social media presence.
  • Perform additional and related duties as required

Requirements:

  • Good communication skills.
  • Good people and organizational skills.
  • Bilingual a plus.
  • Bachelor’s Degree in Social Work or related field a plus.

REGISTERED NURSE (RN) / LICENSED VOCATIONAL NURSE (LVN)

Job Description:

Assess patient health problems and needs, develop and implement nursing care plans, and maintain medical records. Administer nursing care to ill, injured, or disabled participants.  May advise participants on health maintenance and disease prevention and provide case management.  Records on participant’s charts as services are given.  Records initial and final evaluations, makes daily progress notes, and schedules participants for further treatment. Works in close cooperation with staff and participants as a member of the Multidisciplinary Team.

Requirements:

  • Valid California Nursing License.
  • Bilingual a plus.

Job Description:

Requirements:

  • Bachelors Degree in Social Work and 2 years experience providing social services in one or more of the fields of aging, health, or long-term care services.
  • Masters in Psychology, Masters in Gerontology, Masters in Counseling and 1 years experience providing social services in one or more of the fields of aging, health, or long-term care services.
  • Masters of Social Work or Licensed Clinical Social Worker
  • Bilingual a plus.

DRIVER

Duties & Responsibilities:

  • Transport seniors and disabled adults to and from the program and their place of residence. Adhere to traffic and highway safety rules and emergency procedures. Maintain transportation logs. Assist with loading and unloading passengers on vehicles. Interact with passengers in a professional manner, Complete daily maintenance checks on vehicles and notify manager of any issues

Requirements:

  • Valid Class A or B driver license and Passenger Endorsement.
  • Valid Class C Driver License

Job Description:

Assists Program Director with daily operational practices and duties for facilities. Conducts all participant scheduling to include transportation, attendance and handles all participant inquiries. Responsible for dispatching drivers and ensuring efficiency in route management and effective communication between participants, drivers, and staff. Handles all requests for changes timely and efficiently enuring safe and timely arrival and departure for participants. Coordinates all drivers, buses, service of vehicles and routes. Additional duties as assigned may be provided.

Job Description:

Under general supervision, perform custodial services required to maintain facility areas in a clean, orderly, and secure manner.  Keep inventory of assigned materials and items and perform minor, non-technical maintenance related repairs.

Participates in Multi-Disciplinary team meetings reviewing each caseload. Maintains accurate progress notes reflective of daily participation and progression. Develops monthly newsletter and calendar for the center.

Requirements:

  • Bachelors Degree in Recreational Therapy.
  • Certification by the National Council for Therapeutic Recreation Certification preferred.

Job Description:

Job Description:

Responsible for handling high volume calls and logging all calls into computer database. Maintains a high level of professionalism in greeting all visitors and guests and provides outstanding services to all outside inquiries. Maintains accuracy with notations and scheduling. Works closely with Administrative staff on special projects. Assists Transportation Coordinator with transportation scheduling, route coordination, and travel times. Contacts participants to remind of their program participation. Additional tasks and projects as assigned by Administrative Staff.

Assists Executive Administrative Assisitant during absence, with billing, accounts payable and receivables and manages all vendor accounts. Conducts initial and continuing assessments for the development of individual treatment plans. Records on participant’s chart the services given; records initial and final evaluations, makes progress notes and schedules participants for further recreational therapy. Evaluates the participant’s functional status in relationship to participation in recreational activities. Provides for recreational therapy opportunities; instructs family / staff and others on rehabilitation procedures indicated.  Assist participants with personal care as needed.

Job Description:

Maintaining all medical records electronically and physical charts. Conduct utilization review for all medical records, monitoring for content, quality, consistency, and accuracy.  Able to coordinate and develop accurate and detailed communication system across depts.  Responsible for filing all medical records timely.  Assistance to other depts for documentation development.  Responsible for thinning of all medical records timely.  Reports to Program Director findings and corrective action plan.

Requirements:

  • Experience with medical record management, medical abbreviations, and ICD-9/10 Coding a plus.

KITCHEN MANAGER

Job Description:

Maintaining all kitchen area clean and presentable.  Ordering all meals and specialized meals.  Assisting in menu development and planning.  Logging of all meal temperatures upon arrival, delivery and serving.  Maintaining effective communication with contract vendor on needed changes and adjustments.  Maintaining accurate documentation for meal counts, meal items, temperatures, and serving sizes.  Ordering all kitchen supplies and maintaining accurate inventory of items in stock. Reports to Registered Dietician and Registered Nurse. Ensuring accurate and timely delivery of meals to participants.  Maintaining sanitary and practices according to health standards.

Requirements:

  • Detail orientation, Multi-tasking ability, initiative, resourceful.
  • Experience in working in food preparation setting.
  • Good written and oral communication skills, and organizational skills.
  • Bilingual a plus.
